Referrals

Physicians: Please feel free to fax referrals to us at 406-662-7243 or you can email referrals to admin@mindfuldevelopmentoccupationaltherapy.com. 

 

Please make sure all scripts include: ​

  • Child’s name

  • Child's date of birth

  • ICD-10 code for any medical diagnoses (if applicable)

    • If no medical diagnosis, please include suspected ICD-10 therapy codes for OT

  • “Eval & Treat” statement, specifying the appropriate discipline

  • Signature of provider

  • NPI number of provider

Families: Please feel free to have your physician fax/email us a referral, or you can call and set up an evaluation with us and we will take care of the rest!

Billing & Insurance

Please acquaint yourself with the benefits and limitations of your insurance policy since many insurance plans require authorization prior to certain treatments. It is helpful to call your insurance to determine deductibles, copay/coinsurances, number of visits allowed, and if your plan covers Occupational Therapy & Speech Therapy services.

We also offer competitive private pay rates for those without insurance or with out-of-network insurance plans! HSA cards are accepted.

Frequently Asked Questions

Do you accept insurance?

Yes, we accept many plans such as Medicaid, Bluecross Blueshield, Health Montana Kids. If we are not in-network with your insurance, we offer competitive private pay rates and prompt pay discounts to help with costs!

What is your attendance and cancellation policy?

We ask for at least 24 hours notice of cancellation. We ask that you cancel if there is anyone in your family experiencing illness to help keep all patient's and their families healthy. If you cancel 3 sessions without rescheduling, there will be a $40 cancellation charge and you will be removed from the schedule. We are willing to work with you to reschedule on a case-by-case basis. If you are unable to keep up with your attendance, we will happily put you on a flex plan and will call you weekly for cancellations and/or open spots!

How will I be billed for services?

We do all of our billing for the week.  All private pay sessions, co-pays, and co-insurances will be billed at that time. Insurances are billed weekly as well. We ask that all families have a credit card on file and that we bill following your session. This assures you won't have any huge balances building up and no surprises!

What forms of payment are accepted?

We accept cash, check, credit card, and HSA cards.

Where will services be provided?

All services are provided in the home unless a different opportunity for location is determined.

Will an evaluation be required prior to beginning services?

In most instances, yes. If you will be using private insurance or Medicaid, they require medical necessity for pre-authorization of services. If you have recently had a medical evaluation somewhere else, within 6 months, we may be able to use that evaluation. If you are private paying and have recently had an evaluation we are happy to look it over and build our own treatment plan from that recent evaluation. This will all be decided on a case-by-case basis!

Can I leave my child at home with you and run errands?

No, an adult must be present in the home for services to be provided.

What ages do you work with?

We work with all ages of pediatrics, birth-21. 

​

How do I know if my child needs therapy?

Many times you will get a referral from a pediatrician or have a teacher reach out about some challenges that they are observing in the classroom. Or you just have a gut feeling as a parent that your child is displaying difficulty with developmental milestones. Every child grows at their own pace, but getting an evaluation can never hurt! Give us a call and we can chat and see if an evaluation might be helpful at this time!

How long will my child need therapy?

All children have different strengths and needs and blossom in their own time. and we can't always predict when that time will be!

Will you collaborate with my child's teachers/other therapists, etc.?

We absolutely love to coordinate services and collaborate with others on your child's team. We feel this will only benefit your child!

What is play-based therapy?

A child's work is to learn through play. All of our sessions are play-based, child-led, and strength-based. This helps to engage each child to see progress.

What does a parent/caregiver coaching session look like?

When we are working with a child for Early Intervention, we recognize that children can thrive and learn best in their home environment. We will help you learn all of our suggestions---sometimes this can be done by modeling those strategies through play and routines in your home or via teletherapy
